New Student Registration

A parent or guardian must be present at the time of registration and is responsible for providing Bear River Middle School with the following information:

  • Birth Certificate

  • Immunizations

  • Withdrawal form with grades

  • Past Report Card or Transcript

  • Proof of Guardianship

  • Court documents if applicable

    Without the above information we are unable to enroll your student. If your student has not been officially withdrawn from the previous school, you will need to do so before the enrollment process can begin.
